* All Open Water Dive Trainee's are provided with Buoyancy Control Device (BCD) and Regulator as part of there expedition fee. * All volunteers are provided with Scuba Tank, Weights, Weight Belt as part of there expedition fee. All other diving equipment from the equipment list, must be provided by the volunteer or rented from Barefoot Conservation. * Please see our "Costs & Dates" page on our website for more details on rental prices. ** For safety reasons, it is mandatory for all volunteers to wear a Dive Computer during all dives. The volunteer must either bring their own or rent one from Barefoot Conservation. A £150 refundable deposit is needed when renting a Dive Computer.
A £100 non-refundable deposit must be sent with this form to reserve your place on the expedition. Full balance of your expedition fee must be paid a minimum of 8 weeks before your expedition start date.

I agree that I know of no reason, medical or other, why I can not take part in Scuba Diving activities with Barefoot Conservation Ltd. I will provide Barefoot Conservation Ltd with the completed PADI Medical Statement forms signed by my doctor if necessary, a copy of my diving & travel insurance policies and any scuba diving certifications, at least one month before expedition start date.
